Personal data processed
Dana MD B.V. may process personal data about you because you use Dana MD B.V. services, and/or because you provide it to Dana MD B.V. yourself when completing a contact form on the website. Dana MD B.V. may process, inter alia, the following personal data:

Why Dana MD B.V. needs data.
Dana MD B.V. processes your personal data in order to contact you by telephone or e-mail if you so request. In addition, Dana MD B.V. may use your personal data in order to perform a contract of services with you, which usually consists of receiving obstetrical care and/or Integral Pelvic Therapy treatments, attending one of our trainings or workshops, or renting practice or workshop space.
How long Dana MD B.V. keeps data.
Dana MD B.V. will retain your personal data for no longer than is strictly necessary to fulfill the purposes for which your data is collected. Your data will not be kept for longer than one year if no agreement with you is concluded.
Sharing with others
Dana MD B.V. will only provide your personal information to third parties if necessary to perform a contract with you, or to comply with a legal obligation.
Mapping website traffic
On the website of general visit data are recorded, including the IP address of your computer and the time of retrieval and data that your browser sends. This data is used to analyze visiting and clicking behavior on the website. Dana MD B.V. uses this information to improve the operation of the website. Privacy/use of personal data clients who come under obstetric care at Dana MD B.V.
Our practice may process various personal data about you. This is necessary to provide you with good medical care and necessary for the financial settlement of the care provided to you with the health insurance companies when applicable.
In addition, processing may be necessary if we believe your health is at serious risk, or if we need to comply with a legal obligation (for example, mandatory reporting of cases under the Public Health Act). Under the European General Data Protection Regulation (AVG), Dana Center (Dana MD B.V.) is required by law to obtain written consent to process your personal data prior to offering obstetric care. Without your consent, we cannot proceed to offer obstetric care.
By agreeing to this privacy agreement, you agree to:
- The storage and processing of the following personal data in our electronic patient file (EPD): name, address, place of residence, email address, telephone, date of birth, marital status, general practitioner data, health insurance data, Citizen’s Service Number, special personal data(especially in the questionnaire such as data on ethnic origin, health data), your pregnancy data and the data of any previous pregnancies, as well as your partner’s name and contact data if applicable;
- Sharing your information with other health care providers in charge of managing your pregnancy (such as gynecologist, pediatrician, family doctor, maternity care, etc.);
- Exchanging the following information: your name, address, date of birth, BSN, insurance number and what type of care was provided, with your health insurer for the purpose of claiming care provided;
- With using the transmitted email address for midwifery purposes and sending information about our practice.
All employees within Danacentrum will treat your personal information confidentially.
Your personal data are well protected against unauthorized access and are not kept longer than necessary for proper care.
For medical data, the retention period is 20 years (from the last consultation), unless longer retention is necessary, for example for the health of you or your children.
This is at the discretion of the healthcare provider.
If we wish to request data or information about you from (former) practices or health care providers, we will ask you for your express permission to do so, and you will fill out a separate consent form to do so.
GP
We will inform your doctor about your pregnancy. This is important in connection with existing or future use of medication, for example. Data relevant to the supervision of your pregnancy will be exchanged. Your doctor will also receive a message when you have given birth.
Perined and Peridos:
By signing our care agreement, you give Danacentrum permission to provide anonymous information to the agencies Perined and Peridos.
Perined aims to increase the quality of perinatal care in the Netherlands. Professional organizations such as KNOV, LHV, NVOG, NVK and NVVP cooperate in this. Perined deals with audit and registration within birth care as a basis for research and contributing to improving the quality of birth care in the Netherlands. Danacentrum keeps data for this registration, such as the place of delivery, gestational age at delivery and the child’s birth weight. Data is processed anonymously, so pregnant and newborns cannot be traced as individuals. Perined is responsible for ensuring that the data is only used for the stated purposes and is optimally secured. See also www.perined.nl
Healthcare providers who provide information and counseling about prenatal screening or who perform prenatal screening record prenatal screening data in Peridos. This includes the NIPT, the ETSEO and the SEO. The aim is to ensure the quality of screening and use data for scientific research. The system meets strict requirements in terms of privacy of medical data. The data are used almost exclusively anonymously. It is possible to have the data removed from Peridos after the screening is completed. See also www.peridos.nl
Birth Cards
After the birth of your baby, we enjoy receiving a birth card.
A birth card contains personal information.
If you send us a birth announcement card, you agree that it will be displayed in our waiting room or on the notice board of the midwives.
If you do not want this, you can indicate this on the card or make the personal data invisible.
Privacy/use of personal information for clients coming under IPT care at Dana MD B.V.
By agreeing to this privacy agreement, you agree to:
- The storage and processing of the following personal data: name, address, place of residence, email address, telephone, date of birth, special personal data(especially in the questionnaire such as health/pregnancy data etc.);
- With using the transmitted e-mail address to send information about our practice.
- Maintaining an electronic patient record (SPD) containing your personal information, your appointment schedule and records of treatments.
All employees within Danacentrum will treat your personal information confidentially.
